I had an old Llama 380 let go in a similar way years ago. Mine cracked the bottom the barrel and vented gas through the mag well. It bulged the slide, and I had to cut the link to get it apart. Looks like bad metallurgy/heat treating in my case.

We average about one a year...in almost every case I can think of it was reloaded ammo...with the aforementioned exception....bargain basement bullets in a bag bought at a gun show...almost every time....
one time it was ...no kidding ... WWI issue ammo in a beautiful 1911 commercial..very low serial number...bullet stuck in a barrel and then another behind it..managed to get it apart and only the barrel was damaged...but that was enough...and the WWI ammo was in issue boxes...and the gentleman finished the qual with the gun he carried in WWI that he also had with him and also a 1911...was about 98 at the time and still spry and qual'd for his ccw... he's gone now but an amazing gentleman... have also seen people shoot the whole qual course ...50 rounds... by shooting 9mm in a glock 23... Last really good blow up was a baby eagle 40... the guy bought the ammo at a local range and took the pieces back to the shop along with the ammo...last I heard they took care of him...
